Important Medical Information
If your students will need medication to be given during the school day or an overnight school trip, (INCLUDING over the counter medication such as Ibuprofen, Tylenol, Tums, etc…) fill out and have signed by your child’s doctor any of the medical forms that apply.
Please remember medical orders are only valid for 1 year so a new form needs to be filled out and signed EVERY SCHOOL YEAR.
- Turn in forms and medication to the front office medical representative.
- Medication must be in the original correct bottle (with the student’s name/prescription label).
